Exams provide both doctor and patient time for an in-depth assessment and treatment. There are exam options for different situations.
- If you have experienced a major health event or trauma since your last visit, please schedule a Re-Exam. This includes birth, surgery, or accidents regardless of the last time you had an appointment.
- If you are returning to care after 3 or more years away, please schedule an Existing Patient Intake.
- If you are returning to care after 1 – 2 years away, please schedule a Re-Exam.
- If you are returning to care after 6 months to 1 year away, please schedule a Brief Re-Exam.
We have limited spaces for exams. If you need to be seen sooner than the first available appointment, please contact us. You can also place yourself on a waitlist when scheduling.
Once your exam is scheduled, you will receive an update form via email. If you are 65 or older, you may also be emailed an ABN form. Please complete online forms no later than 24 hours before your appointment. If we do not receive your forms in time, we may need to reschedule your exam.
