For adults new to our practice, please schedule a New Patient Intake.
For children under 18 years old new to our practice, please schedule a New Patient Intake Age 17 and Under.
We have limited spaces for exams. If you need to be seen sooner than the first available appointment, please contact us. You can also place yourself on a waitlist when scheduling.
Once your initial exam is scheduled, you will receive an intake form via email. If you are 65 or older, you may also be emailed an ABN form. Please complete online forms no later than 24 hours before your appointment. If we do not receive your forms in time, we may need to reschedule your exam.
